If known, give the book & page number and the type of document being requested. If you do not know the book and page number, give as much information as possible such as the date of recording, parties involved, document control number, address of the property, and the type of document. • Please indicate whether or not you are requesting a certified copy. • Complete the requestor information section. Copy charge is $.25 per page. The certified copy charge is $5.00, if over 10 pages there is an additional $2 charge. A postage and handling fee of $2.00 is charged for each document requested. Our office accepts cash, check, certified check, business check, or money orders.
